Achievement of correct knee alignment is crucial to ensure the best outcomes following a procedure to re-align the knee. The use of customized surgical instrumentation for individual patients to assist in preoperative planning and intraoperative realignment has gained increasing popularity. The purpose of this article is to describe a surgical technique involving a knee re-alignment procedure (high tibial osteotomy) performed using three-dimensional (3D) customized instrumentation to revise lower extremity mal-alignment. The surgical considerations and details are planned preoperatively using virtual software and advanced imaging to allow precise surgical execution.
